I hereby submit the following application form for the use of the Design Fabrication Center facilities. The following terms will be strictly observed.
1. All rules will be strictly observed and security of the facility will be maintained．
2. All equipment and furnishings will be handled with care, and all aspects of the facility will be returned to the original state after use.
3. Student who do not have Gakkensai (Personal Accident Insurance for Students Pursuing Education and Research) and/or Gakkenbai (Personal Liability Insurance for Students) will not be permitted to use any equipment.
4. All equipment users will be required to undergo the necessary safety tutorials prior to use.
